Refactor create-env

Make create-env more configurable, narrowing its focus to test CI jobs
specifically. The separation of test configuration and runtime
configuration is made more clear.

The generated output has also been improved (the resulting Vagrantfile
should work with both libvirt and Virtualbox without further changes).
